If you are working online only and do not include any mentionings of a location, you are putting yourself in competition with over 1 billion search results for sites targetting 'online therapy' for example (which would of course be extremely difficult to rank anywhere for). We therefore always recommend including specific areas (the more specific, the easier to rank well for), so that your site can be found by people.
